How the GUIDE Program Supports Families

People with dementia frequently need extensive caregiver support, and it’s typically the spouse, adult child, or another family caregiver performing the care. As rewarding as it can be, family members can bear mental, emotional, physical, and financial strains due to caregiving. A lack of support can result in inconsistent, poorly coordinated care, which can increase health risks for both the caregiver and their loved one with dementia.

Caregivers typically report high stress levels and depression. At Village Caregiving, we understand that you, as a caregiver, need to take care of yourself, too. We’re a home care provider that accepts the Guiding an Improved Dementia Experience (GUIDE) program in each of the states where we offer services. We can provide the support you need when taking care of your loved one.

What Is the GUIDE Model?

The GUIDE model is a voluntary program set by the Centers for Medicare & Medicaid Services (CMS) that aims to study the effects of comprehensive services and support for people diagnosed with dementia, as well as their caregivers. The goal is to enable people with dementia to live in their residences longer by delaying or preventing nursing home placements, reducing Medicare and Medicaid expenses and improving their quality of life. It contributes to the National Plan to Address Alzheimer’s Disease, set by the National Alzheimer’s Project Act.

The program standardizes evidence-based services, which include:

  • Comprehensive care assessments and planning.
  • 24/7 access for caregivers.
  • Home visits.
  • Ongoing monitoring.
  • Caregiver education, training and support.
  • Care coordination.
  • Transitional care management.
  • Medication management.
  • Referral to and coordination of community-based services.

The program started on July 1, 2024, and will run for eight years. Eligible participants with an unpaid caregiver get access to up to $2,625 in respite services for 2026 — a cap that gets adjusted annually. They also get care management payments for services, which are based on the patient complexity, caregiver burden and location.

The GUIDE program improves home care services by ensuring services are accessible, thanks to the reimbursable payments and screening. Home care agencies that accept the GUIDE program need CMS approval.

How Does the GUIDE Model Work?

People with dementia must give consent to receive GUIDE services from a GUIDE participant, who is a physician or a health institution. This acceptance is called alignment, and each patient can only be aligned to one GUIDE participant at a time. The GUIDE participant submits your loved one’s information to CMS. You, as a caregiver, get pulled to the same alignment and are treated as a co-recipient of the services. Note that the alignment doesn’t restrict your loved one from receiving care services from other providers.

GUIDE Program Eligibility

People with Medicare Advantage or those who receive the Medicare hospice benefit cannot enroll. In Medicare Advantage, it’s the private insurers who manage and pay for a person’s care instead of Medicare. Meanwhile, Medicare hospice services overlap with the services offered in the GUIDE model.

Your loved one with dementia can enroll in the GUIDE program if they:

  • Are diagnosed with dementia.

  • Have Medicare Parts A and B as their insurance.

  • Live in a private residence or an approved community-based residential community, not a long-term care facility.

We also work with families to help make our services as affordable as possible. Our services come with a flat rate and simple paperwork, plus there are no hourly minimums or deposits. We also offer flexible payment options, whether you use Medicare or Medicaid.

Note that we’re not a GUIDE participant — that pertains to your healthcare provider, who can connect you and your loved one to the right clinical services and other community-based resources as needed. Our role lies in providing the in-home care your loved one needs, so you can also take care of yourself as a caregiver.
